Scheduling Form Process

If you are ready to schedule your appointment:

- Fill out and submit the following request form.

- The office will review your request and email an invoice.

- The invoice will need to be paid to book the appointment.

- – On the day of your appointment, we’ll be in touch with a more specific arrival window as soon as our schedule is set, and we’ll text you when we’re on the way.

After your pet has passed, you’ll choose how you’d like their remains handled. Our team will respectfully prepare your beloved companion and arrange transportation to a trusted crematory. We explain all options clearly, handle every step with dignity, and support your family through the process.

Ashes Picked Up By Family - Your pet is cremated privately and the ashes are picked up by you. Pricing depends on weight: $295–$445

Ashes Scattered Your pet is cremated respectfully and ashes are not returned. Pricing depends on weight: $200–$350

Ashes are available within 1-3 business days. Pick up locations are Goshen, Loveland or Fairfield.
